Credit Evaluation
To have credits you earned at another school considered for transfer to LFCC, complete the Transfer Credit Evaluation Request form. This form is not yet available online. You can obtain this form from the Admissions and Records Office.
Curriculum Admission Form
The curriculum admission form identifies the program of study you are in. It may mirror your application and is used to identify any changes. You should complete the curriculum admission form to confirm curriculum admission.
