Transfer Credit Evaluation Request form

If you have completed course work at another college or university and want to receive transfer credit at LFCC, follow these instructions:

• Submit a completed application for admission
• Have official transcripts sent directly from the school(s) you attended to the Admissions and Records Office
• Provide course descriptions from the schools to the Admissions and Records Office
• Submit a Transfer Credit Evaluation Request Form.
